Care Where It’s Needed
Our mobile clinical teams provide on-site medical evaluations, primary and acute care, preventive screenings, chronic condition follow-up, and care coordination. We serve individuals who may face barriers to accessing consistent healthcare and connect them with the resources and services they need.
Beyond the Visit
We believe healthcare shouldn’t end when the visit does. Through our street medicine program, Rafi Health helps patients navigate the healthcare system, access follow-up services, and connect with ongoing primary and specialty care. We work to build continuity of care and ensure patients have the support and resources they need beyond a single encounter.
Healthcare shouldn’t depend on a patient’s ability to get to a clinic. We bring the clinic to the patient.
